Testis development is usually complete after 30 -32 weeks of gestation and it is usually taking place in the abdomen. At birth 70% of the male babies, the testis has descended into the scrotum. Very less number of kids or the testis is undescended, in 6 months to 1 year they may descend back to the scrotum. If it is not descended and if it is not treated after 6 months, then this will affect the germ cell maturation inside the testis. When the child is becoming an adult, the number of spermatozoa which sis present in the semen will be lesser and this may result in infertility. And undescended testis if not treated, it can lead to testicular torsion, that is twisting of the testis. Also they have a high incidence of testicular cancer. So undescended testis is testicular torsion considered as an emergency in a male child and it has to be treated so that infertility does not happen later on in life.

Underdeveloped genitals like testes if they are undescended it leads to infertility. First we have to see this testes has to come down as early as possible as age into the scrotal region because the scrotum protects the testes by keeping the testes in slightly lower temperature than normal. If it is in the abdomen the temperature is higher which causes the testicular tissues to be damaged and cause infertility. So the earlier you operate, get the testes normal before 6 months or 2 years we usually recommend. The testes is brought down before two years the fertility will be preserved. The more later you do the fertility is destroyed. It is not only about fertility, the later it is brought down the chances of testicular tumour is higher in these cases.

It was unrelenting. Tylenol didn't help. A warm soak didn't help. Finally, his mom decided to take him to the emergency room. First he was seen in triage. Then he was seen by a doctor in training. Then they needed to call in the senior attending doctor. Then they decided to call in a pediatric urologist. Then they decided he needed a special ultrasound test called color Doppler flow. Only one doctor in training recognized the possibility that this young boy had a condition known as testicular torsion. That means that the testicle twisted upon itself. If not untwisted in a timely fashion, the blood supply to the testicles can get cut off. This will create a dead testicle, also known as a necrotic testicle. If the testicle has died, a surgeon will need to go in and surgically remove the testicle. By the time the appropriate doctors were called in to see this young boy and by the time he had a special ultrasound to evaluate whether there was still blood flowing to the testicle, it was clear that they had lost their window of opportunity to reverse this problem. Select patients with adult-onset idiopathic hypogonadotropic hypogonadism may respond to clomiphene citrate therapy. Prader-Willi syndrome: Patients have characteristic obesity, mental retardation, small hands and feet, and hypogonadotropic hypogonadism due to a GnRH deficiency. Prader-Willi syndrome is caused by a disorder of genomic imprinting with deletions of paternally derived chromosome arm 15q11-13. Laurence-Moon-Biedl syndrome: Patients with this syndrome have retinitis pigmentosa and polydactyly. Infertility is due to hypogonadotropic hypogonadism.
